Home Health Registered Nurse (RN)
Schedule: Monday–Friday, 8:00 AM–5:00 PM (some weekend availability preferred: ~1 weekend/month) + ~1 on-call night/week


Location: Pensacola, FL 32514 (covers Escambia, Santa Rosa, Okaloosa, Walton counties) — On-site/in-the-field home health

Home Health Registered Nurse (RN) Summary: Home Health RN / Case Manager supporting high-risk members through in-home visits. You’ll complete comprehensive assessments, build and manage individualized care plans with providers, coordinate transitions of care, document clinical/functional outcomes, and serve as a patient advocate.

Home Health Registered Nurse (RN) Requirements:

  • Active RN license (Florida required; compact accepted per job notes)
  • 1+ year Home Health RN experience
  • OASIS experience (required)
  • Homecare Homebase / PointCare (required)
  • Comfortable with home health productivity expectations (approx. 4–6 visits/day; 20–25 patients/week)
  • Preferred clinical skills: wound care, PICC/IV care, lab collection, drain care, ostomy care, post-surgical care
