Behavioral Health Specialist โ€“ Full Time

Location: Chicago, IL
Schedule: 5 Days/Week | Hybrid (In-Clinic & Telehealth)
Patient Volume: 1 Patient Per Hour

What Youโ€™ll DoComprehensive Outpatient Care
  • Conduct mental health assessments and develop individualized treatment plans.
  • Provide outpatient therapy to adults and/or children.
  • Deliver crisis intervention and behavioral stabilization as needed.
  • Maintain a structured caseload of one patient per hour to ensure quality care.
Integrated Team Collaboration
  • Work side-by-side with Primary Care Providers within our MindPATH model.
  • Collaborate with care managers and interdisciplinary teams.
  • Connect patients with appropriate community and behavioral health resources.
Clinical Documentation & Monitoring
  • Maintain accurate treatment plans and progress notes in the EHR.
  • Monitor patient outcomes and adjust interventions as needed.
  • Prepare discharge planning and case summaries.

What You BringRequired:
  • Masterโ€™s Degree in Social Work, Counseling, Psychology, or related field.
  • Active Illinois license in one of the following:
    • LCSW (Licensed Clinical Social Worker)
    • LCPC (Licensed Clinical Professional Counselor)
  • Minimum 3 years of experience providing outpatient behavioral health services.
  • Experience with assessments, treatment planning, therapy, and crisis intervention.
  • Ability to practice independently in the state of Illinois.
Preferred:
  • Experience in integrated or primary care behavioral health settings.
  • Comfortable collaborating with medical providers on medication management.
  • Bilingual (English/Spanish) strongly preferred.
  • Experience working with underserved or vulnerable populations.
