Position Overview
We are seeking a motivated and compassionate Case Manager (QMHA) to join our growing team. This role focuses on supporting clients in accessing resources, building stability, and navigating systems of care.
This is a full-time position, consisting of:
● 25–30 client contacts per week
● Additional administrative/documentation time
One of the unique benefits of this role is the ability to create your own schedule, allowing for flexibility and autonomy in your workweek.
Key Responsibilities
● Provide case management services in alignment with QMHA scope
● Connect clients with community resources (housing, medical care, benefits, etc.)
● Collaborate with clinicians and multidisciplinary team members
● Support clients in achieving treatment plan goals
● Maintain accurate, timely documentation in the EHR
● Advocate for clients and reduce barriers to care
● Conduct outreach when appropriate (community-based work may be included)

Qualifications
● QMHA (Qualified Mental Health Associate) certification required (or ability to obtain upon hire, depending on candidate)
● Bachelor’s degree in social work, psychology, or related field (or equivalent experience aligned with QMHA requirements)
● Experience in behavioral health, social services, or community-based work preferred
● Strong organizational and communication skills
● Ability to work independently while collaborating with a team
● Knowledge of community resources is a plus
