We offer a variety of programs including inpatient and outpatient behavioral health, residential treatment for court-involved teenage girls, geriatric services, clinical day treatment schools and chemical dependency services. Job Summary
The Case Manager is responsible for providing a range of services for clients with either Psychiatric and/or Substance Abuse issues. Also provides advocacy, outreach, and service linkage for clients and initiates access to entitlement services. Works in coordination with interdisciplinary teams of the Adult Partial Hospitalization Program.
Qualifications:
• Experience working with adult behavioral health and/or chemical dependency population
• Bachelor's degree in Human Services or related field; Social Work preferred
• Valid driver's license may be required based on program
• Knowledge of community resources preferred.
• Computer proficiency required
