We are seeking a dedicated and experienced Case Manager to lead our Developmental Services Program! This role is ideal for a seasoned Case Manager with a background in developmental services who is passionate about delivering high-quality, person-centered care for individuals with developmental disabilities.

What You'll Do:

The Case Manager is responsible for coordinating and monitoring service delivery for individuals served in Support Services by completing case management functions, facilitating person-centered planning, and ensuring required documentation and communication with internal and external stakeholders.

Responsibilities include:

  • Initiating, supporting, and promoting quality of life for clients through empowerment, safety awareness, respect, and dignity.
  • Coordinating and participating in Health Care Management meetings, accompanying clients to medical appointments, arranging necessary follow-up appointments, and collaborating with Staff Supervisors and the nursing staff.
  • Coordinating medications-related documentation in accordance with agency policy and MAP regulations.
  • Providing crisis intervention and emergency services as needed (including participating in the on-call process as needed).
  • Recognizing and reporting concerns related to abuse/neglect, health, and safety.
  • Completing all documentation required in the client's electronic health record.
  • Completing case management forms and other required documents in a time-sensitive manner.
  • Other tasks as required by management

What You'll Need:

  • Associate's Degree (or equivalent) OR High School Diploma / GED AND 3 years of relevant experience (Bachelor's Degree preferred)
  • 2+ years of experience in human services
  • Experience with case management, documentation, and coordinating services in a human service setting
  • Strong verbal and written communication skills
