Job description

Summary
The Diabetes Care & Education Specialist RN (DCES) provides expert inpatient and outpatient education for individuals with diabetes in alignment with Parkview Health Standards of Care and American Diabetes Association (ADA) accreditation guidelines.
This role integrates clinical expertise with self-management education to promote optimal health outcomes for patients with diabetes and related comorbidities. The DCES collaborates with patients to establish individualized goals, supports behavior change, and promotes lifelong self-management to improve quality of life.
Responsibilities include:
  • Delivering evidence-based diabetes education and clinical care.
  • Reviewing medications for therapeutic effectiveness.
  • Collaborating with interprofessional teams to develop and evaluate patient-centered care plans.
  • Documenting all patient interactions accurately and promptly in the EMR.

Education
  • Associate of Science in Nursing (ASN) - Required.
  • Bachelor of Science in Nursing (BSN) - Required within 3 years of start date.

Licensure & Certification
  • Current RN License - State Board of Nursing.
  • Certified Diabetes Care and Education Specialist (CDCES) or Board Certified in Advanced Diabetes Management (BC-ADM) - Preferred.
  • Current CPR certification - Required.

Experience
  • Minimum 5 years of RN experience in a clinical setting - Preferred.

Qualifications
