Licensed Practical Nurse (LPN) Responsibilities
The Licensed Practical Nurse (LPN) is responsible for, but not limited to:
Observe and document the physical, emotional, and psychosocial needs of patients and their families.
Collaborate with the Hospice RN regarding patient status, symptom management, and changes in condition.
Provide skilled nursing care as directed by the attending physician and supervised by the Hospice RN.
Monitor patient responses to treatment and document all care provided.
Maintain continuity of care through effective communication with patients, families, physicians, and the interdisciplinary team.
Participate in Interdisciplinary Team (IDT) meetings and other required clinical meetings.
Maintain adequate home care supplies to support quality patient care.
Assist with collecting clinical documentation and required paperwork.
Support clinical billing activities as needed.
Assist with the orientation and training of new Hospice Aides when assigned.

Education
Qualifications
Current, unrestricted LPN license in the state of practice.
Minimum of one (1) year of clinical nursing experience.
Hospice, Home Health, or Long-Term Care experience preferred.
Strong verbal and written communication skills.
Ability to work effectively as part of an interdisciplinary team.
