Position Overview

We are seeking a dedicated and skilled Physical Therapist (PT) to join our team at Lifepoint Therapeutics. Work Environment

This position is based in an onsite home care setting, allowing you to work directly in clients' homes across the Chantilly area. As part of our interdisciplinary team, you will collaborate closely with other healthcare professionals to deliver comprehensive treatment tailored to each client's needs.

Key Responsibilities
  • Develop and implement individualized physical therapy treatment plans aimed at preventing disabilities, relieving pain, and restoring optimal function in clients, taking into account their unique capabilities and goals.
  • Supervise Physical Therapy Assistants (PTAs) to ensure adherence to the established plan of care and the highest standards of treatment delivery.
  • Conduct thorough evaluations of clients' functional abilities through diagnostic assessments, encompassing muscle, nerve, joint, and overall functional performance.
  • Assess clients' home environments and recommend modifications that promote independence and enhance the rehabilitation process.
  • Deliver prescribed physical therapy services, including administering manual and non-manual therapeutic exercises, traction, massage, and the application of various physical agents.
  • Fit and adjust prosthetic and orthotic devices, collaborating with physicians to recommend modifications when necessary.
  • Maintain comprehensive clinical documentation, including initial evaluations, progress notes, and discharge summaries, to communicate effectively with the care team and ensure continuity of care.
  • Participate actively in interdisciplinary team meetings, providing insights on client progress and contributing to holistic treatment planning.
  • Train and educate clients and their families on the proper use and care of therapy devices, as well as on their individual physical therapy regimens.
  • Stay accessible to PTAs providing treatment and ensure their work aligns with best practices as mandated by applicable state laws.
  • Complete additional assignments and duties as assigned, focusing on achieving the overall goals and objectives of Lifepoint Therapeutics.
Patient Population / Client Focus

This position targets a diverse patient population, including individuals recovering from surgery, managing chronic conditions, or requiring rehabilitation after hospital stays. The focus is on delivering care to clients in the environment where they feel most comfortableโ€”at homeโ€”offering them autonomy while facilitating their recovery.

Qualifications
  • A Doctorate in Physical Therapy (DPT) from an accredited institution.
  • An active PT license in Virginia, free from sanctions or board actions.
  • Strong proficiency in manual therapy techniques and the ability to design effective, evidence-based exercise programs for a variety of conditions.
  • Familiarity with electronic medical record (EMR) systems, particularly Web PT, for efficient documentation and communication.
  • Updated knowledge of current physical therapy practices backed by research and clinical guidelines.
Preferred Experience
  • Experience in home health care or a similar setting is preferred but not mandatory.
  • Previous supervisory experience with PTAs is considered a plus.
