Job description

Position Summary:
Eventus WholeHealth is building an internal project management function to support the operational and technology initiatives driving our next phase of growth. This role will serve as the company's dedicated Project Manager, responsible for leading major cross-functional projects from initiation through completion - with clear ownership of the plan, the timeline, and the outcomes.
The immediate and primary focus will be our migration to Athena Health (athenaOne), a complex, multi-workstream effort spanning clinical documentation, scheduling, billing, and care coordination across our long-term care and skilled nursing facility (SNF) network. The Athena migration is the flagship initiative for this role. As it progresses and matures, this person will take on additional high-priority projects across the organization - becoming a standing resource for structured project leadership at Eventus.
This is not a coordination role. The PM owns the plan, drives accountability, and is expected to operate with a high degree of autonomy. They will report directly to the VP of Operational Excellence and will work closely with clinical, operational, IT, and vendor teams across the company.
Managing the human side of major transitions is central to this work. Eventus has a distributed workforce across multiple SNF markets, and this person will play an active role in workforce communications, readiness planning, and change management on every project they lead.
Key Responsibilities:
  • Own and manage the master project plan, workstream roadmap, and milestone calendar for assigned initiatives.
  • Lead the Athena Health EHR migration, coordinating clinical, operational, IT, revenue cycle, and vendor workstreams.
  • Break down complex initiatives into actionable tasks with clear owners, timelines, and success criteria.
  • Facilitate regular workstream meetings, cross-functional stand-ups, and executive updates.
  • Track project dependencies, risks, and issues, maintaining a live risk and issue register with mitigation plans.
  • Identify and escalate risks proactively to avoid delays or operational disruptions.
  • Serve as the primary point of contact with vendor project management teams, ensuring alignment to timelines and deliverables.
  • Hold vendors accountable to agreed-upon commitments, SLAs, and project milestones.
  • Coordinate third-party system integrations in partnership with IT and external vendors.
  • Develop and execute change management plans to support workforce readiness and adoption.
  • Partner with HR and Operations to deliver clear, timely workforce communications for impacted teams.
  • Coordinate training plans and readiness assessments ahead of system go-lives.
  • Maintain complete and accurate project documentation, including decisions, meeting notes, and governance artifacts.
  • Support post-go-live stabilization activities, including issue tracking, resolution follow-up, and lessons learned.

Required Qualifications:
  • 5+ years of project management experience, with a track record of leading complex, multi-workstream initiatives to completion.
  • Direct experience managing EHR implementations or migrations. This is a firm requirement.
  • Ability to own a project plan and drive accountability across internal teams and external vendors without relying on positional authority.
  • Experience leading change management and workforce communications as part of technology or operational transitions.
  • Proficiency in project management tools such as Smartsheet, MS Project, Asana, or similar; comfortable working in both Agile and waterfall environments.
  • Clear, concise written and verbal communication skills across a range of audiences - from frontline staff to executive leadership.
  • Able to operate effectively in a fast-moving environment where priorities can shift and ambiguity is the norm.

Preferred Background and Knowledge:
  • Direct experience with Athena Health (athenaOne).
  • Background in long-term care, skilled nursing, post-acute, or value-based care.
  • PMP, CAPM, or equivalent certification.
  • Familiarity with healthcare interoperability standards such as HL7 and FHIR.
  • Experience managing technology or operational transitions in distributed, multi-market healthcare organizations.
