Ascendo Healthcare Staffing is actively seeking an experienced Home Health Registered Nurse, RN for an exciting 13-week contract opportunity near Whitman County, WA! This opportunity is moving quickly, and interviews are being scheduled immediately!

NOTE: This position is specifically for a skilled Home Health RN. Candidates whose experience is limited to Home Care, private duty, sitter care, continuous care, or non-skilled support work may not be the right fit. Qualified candidates must have experience with clinical assessment, case management, OASIS, and home health documentation.

POSITION DETAILS

Compensation:

  • Earn up to $128,960 per year PLUS Mileage Reimbursement
  • Weekly Pay: Via Direct Deposit Every Friday Morning
  • Benefits: Medical, Dental, Vision & More (Single & Family Plans Available)

Schedule:

  • 5x8 Day Shifts (8:30 AM – 5:00 PM)
  • On-Call Expectations: Required if requested
  • Productivity Expectations: 30 Units Per Week =40 HRS
  • Note: Our client is looking for Home Health RN's that can maintain a full caseload of 30 productivity points per week. No more guessing! Full-Time Hours!

RESPONSIBILITIES

  • Complete comprehensive patient assessments and develop individualized care plans
  • Manage a home health caseload while maintaining quality outcomes and productivity standards
  • Perform Start of Care (SOC), Resumption of Care (ROC), Recertifications, and OASIS documentation
  • Coordinate care with physicians, therapists, caregivers, and interdisciplinary teams
  • Provide patient and family education regarding disease management and treatment plans
  • Monitor patient progress and communicate changes in condition appropriately
  • Participate in on-call triage rotation as assigned
  • Maintain accurate and timely electronic documentation utilizing Homecare Homebase
