To coordinate Transfer Center transfers from the initial call through patient discharge and follow-up, providing the highest quality service to physicians, nurses, patients and other medical professionals.

  • Minimum Requirements
    • Education: Associates Degree in Nursing, or equivalent, from an accredited program.
    • Experience: 4 years clinical experience in an acute care setting.
    • Licensure/Certifications: Current Arizona, or other compact state, Registered Nurse license; BLS.
  • Preferred Requirements
    • Education: Bachelor of Science in Nursing
    • Experience: RN experience in an ER/Trauma, Neurosciences and/or Cardiovascular setting. Knowledge of, and the ability to apply InterQual criteria.
    • Certifications/Registration: ACLS; Applicable professional certification i.e. CCRN, CEN, CNRN
  • Special Skills:
    • Must possess the ability to interact and communicate in English, both orally and written, with all levels of staff, physicians, patients, and families in an effective manner, exhibiting tact, enthusiasm, and patience.
    • Must be sensitive to cultural and bilingual issues.

  • Respond to all referrals to the Transfer Center program in a timely manner
  • Contact the receiving nurse to outline the patient's condition, medical treatment plan and physical orders to prepare him/her for the patient's arrival
  • Assist with development and implementation of educational programs for nurses (i.e. Lunch & Learns, ACNS, etc.)
  • Facilitate physician-to-physician communication
  • Communicate with sending facility to assure follow-up with Physician accomplished
  • Assist sending facility with use of air or ground transport of patient
  • Telephone receiving facility Admitting Department to admit patient
