Position Summary - RN or LPN

Days 7:00am- 3:30pm, Evenings 3:00pm-11:30pm, or Nights 11:00pm-7:30am - Rehabilitation and Skilled Nursing

The Staff RN/LPN is responsible for implementing, directing, communicating and documenting the nursing process and provides a safe environment for patients, visitors and associates. The RN/LPN practices in accordance with current established standards of care, the Nurse Practice Act, the Powers Health policies and procedures.

Job Responsibilities

  • Documents a physical assessment each shift including subjective/objective data.
  • Communicates with patients, families and/or significant others in an effort to individualize patient’s plan of care to them.
  • Verifies that physician’s orders are transcribed properly and carried out promptly.
  • Reports significant changes in patient’s condition to charge nurse / shift supervisor and/or physician.
  • Continuously evaluates patient’s responses to plan of care, teaching plans and rehabilitation activities throughout their stay and upon discharge.

Education/Experience

  • LPN or RN degree from an accredited school of nursing (2 or 4 year program).
  • Currently licensed as a RN or LPN by the State of Indiana Health Professions Bureau.
  • Obtains and maintains current certification in BLS, and other required certificates in specialty areas.
