As a Hospice RN – Regional Support, you'll provide direct patient care while serving as a floating clinician assigned to hospice branches based on census needs, growth opportunities, or staffing support.

  • Developing and implementing individualized care plans
  • Educating and supporting patients, families, and caregivers

As a Regional Support Hospice RN, You Will:

• Deliver high-quality, compassionate nursing care for hospice patients• Manage and implement individualized plans of care• Support and lead interdisciplinary team members (LPNs, CNAs, HHAs, etc.)• Assess and address patients’ physical, emotional, spiritual, and environmental needs• Accurately document all care provided and ensure compliance with hospice protocols• Educate families and caregivers on care instructions, medications, and safety• Participate in the on-call rotation and provide after-hours care as needed• Collaborate with physicians, referral sources, and the care team• Assist with referrals, regulatory documentation, and payer compliance• Travel to various Gentiva hospice locations as assigned to fill urgent needs

Schedule & Travel:

This role typically follows a Monday–Friday schedule, with occasional on-call responsibilities. Travel is required and may include:

  •  Regional travel with overnight stays based on assignment needs.

A company fleet car is provided for eligible employees.

About You

Qualifications – What You’ll Bring:

• Current, unencumbered RN license in the state of employment• Minimum of 1 year of RN experience (hospice, home health, acute care, long-term care, oncology or other high acuity setting)• Current CPR certification• Valid driver’s license, auto insurance, and reliable transportation

Preferred Experience (Not required):

• Hospice and end-of-life nursing experience• Skilled in clinical assessments and compassionate communication• Strong critical thinking, flexibility, and problem-solving skills• Comfortable working in varied environments with shifting needs• Proficient in documentation and electronic health records• Calm and supportive in crisis situations; skilled at family engagement
