Mindzen Psychiatry is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our compassionate team providing remote care for patients at our Hospice Center in Pomona, California. This role is primarily remote, allowing you the flexibility to engage with clients in a manner that suits both their needs and your schedule. You will work closely with a team of skilled therapists, utilizing innovative therapy modalities in an outpatient setting designed specifically for individuals and families dealing with terminal illnesses and related stressors. Our Hospice Center provides a welcoming atmosphere where team collaboration flourishes, and patient care remains the top priority.

• Deliver evidence-based, culturally competent psychotherapy tailored to meet the unique needs of each client, ensuring a responsive and adaptive therapeutic approach.
• Collaboratively develop and implement treatment plans that align with clients' personal goals, values, and experiences, empowering them in their healing journey.
• Assist clients in navigating complex emotional landscapes including trauma processing, coping strategies for stress, managing depressive symptoms, overcoming anxiety, and addressing relationship challenges.
• Ensure meticulous and timely documentation using our sophisticated electronic health record (EHR) system, meeting compliance and quality assurance standards.
• Engage actively in team case consultations, training workshops, and peer support circles to promote collective growth and learning within our team.
• Opportunities to lead group therapy sessions, contribute to outreach initiatives, and participate in the development of new programs to further our patient care mission.

This role focuses specifically on patients receiving hospice care, which includes actively listening to their stories and understanding their emotional and psychological needs. You will support clients and their families in dealing with the complexities associated with terminal illness, ensuring they have access to mental health resources that are sensitive to their circumstances.

• Active California LCSW license; applications from associate-level candidates nearing licensure are also encouraged.
• A Master’s Degree in Social Work (MSW) from an accredited program.
• Minimum of 2 years of clinical experience in outpatient settings, community mental health care, or related fields.
• Demonstrated commitment to culturally sensitive, trauma-informed, and strengths-based practices.
• Exceptional communication skills, coupled with high emotional intelligence to effectively connect with clients.
• Adeptness at working both independently and as part of a collaborative team.

• Experience working with terminally ill patients and their families is highly desirable.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• History of facilitating support groups or community outreach initiatives.

We offer a competitive salary ranging from $90,000 to $100,000 annually, commensurate with experience and credentials. In addition to the base salary, our benefits package includes:
