Job Title: Physician (MD) – Senior Living Rounding | Primary Care & SNF

Company: Twin Cities Physicians

Compensation: $250,000 to $325,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Position Summary

As a Senior Living Rounding Physician with Twin Cities Physicians, you'll become a trusted medical presence for residents, caregivers, and families navigating complex healthcare situations.

You'll manage patients across assisted living, skilled nursing, memory care, and post-acute settings while helping families better understand conditions, treatment plans, and long-term care goals. Your expertise won't simply impact diagnoses and medications. It will provide reassurance, guidance, and stability during some of life's most challenging moments.

Responsibilities

• Provide on-site primary care and rounding services throughout assisted living, memory care, skilled nursing, and long-term care communities.

• Manage chronic disease, preventative care, transitional care, and post-acute medical needs.

• Perform diagnostics, treatment planning, medication management, and follow-up care.

• Collaborate with Nurse Practitioners, Physician Assistants, nursing staff, therapists, and interdisciplinary teams.

• Support residents and families through complex medical decision-making and care planning.

• Maintain accurate EMR documentation and coordinate ongoing care needs.

Qualifications

• Active Minnesota Physician (MD or DO) license in good standing.

• Experience in geriatrics, internal medicine, family medicine, SNF care, post-acute medicine, or related specialties preferred.

• Strong clinical judgment and confidence managing medically complex patient populations.

• Excellent communication skills and ability to build trust with residents and families.

• Valid driver's license and reliable transportation.
