Greater Nashua Mental Health is seeking a compassionate and motivated Case Manager to join our Assertive Community Treatment (ACT) team. In this role, you'll work directly with individuals experiencing severe and persistent mental illness, helping them access services, build independence, and achieve recovery goals in their own communities.

Why This Role Matters
As an Emergency Services Clinician, you will be on the front lines of behavioral health crisis response, supporting individuals and families during some of their most challenging moments. Your work will directly contribute to community safety, access to care, and positive outcomes for those experiencing mental health emergencies.
Key Responsibilities
  • Provide case management and direct support services to individuals with complex mental health needs
  • Work within a multidisciplinary ACT team to coordinate treatment plans and services
  • Support clients with medication monitoring, crisis intervention, and daily living skills
  • Advocate for clients and connect them with community resources
  • Deliver services in community and natural environments to support recovery and independence
  • Assist with daily medication runs on a rotating schedule
  • Participate in a rotating on-call schedule 1x per week
Qualifications
  • Bachelor's degree in Social Work, Psychology, Human Services, or related field
  • 1-2 years of related experience preferred
  • Strong assessment, diagnostic, crisis intervention, and de-escalation skills.
  • Strong communication, collaboration, and problem-solving skills
  • Ability to work in a fast-paced, team-based environment
