Job Information

Title: Bilingual (English/Spanish) Health Promotion Program Navigator

Department: Community Health & Prevention
Status: Hourly

Position Classification/Category: Clinical
Location: Kintegra Family Medicine - Asheboro
Reports To: Program Manager

Summary of Position

The Health Promotion Navigator position will be primarily responsible for providing patient navigation and care coordination for patients interested and eligible for available grant-funded programs. He/She will provide patient support and linkage to care, in an effort to maintain, improve and adopt healthy lifestyles. He/she assists patients with navigating the health care system by providing individualized education, resources, referrals and advocacy for clients and their families. Works with the Program Manager and the Department Director, assisting with the Community Health & Prevention Department programs. This would include but is not limited to: Work with patient(s) to link to program services and follow-up accordingly, Track measurable outcomes, and Report to partner agencies. It is imperative that this role be able to develop and implement strategies to increase screenings, linkage to care and utilization of care and treatment for patients served. This position is grant funded.


Performance Requirements

  1. Ability to read, write and understand the English and Spanish language
  2. Must have ability to work well within a diverse team. Includes but is not limited to: communicating with community partners, managing data dissemination and coordinating project-related strategies with the team
  3. Must exhibit excellent interpersonal and communication skills, both oral and written (i.e. customer service, telephone skills, and e-mail communication), which are required for interacting with program team, program participants, organization staff and healthcare team
  4. Must follow established guidelines for assessing compliance with protocol and operational procedures.
  5. Communicate effectively with a wide range of patients, operations staff, providers and other departments in a friendly, professional manner incorporating a team-based approach to healthcare – working with the physical and emotional development of all age groups
  6. Knowledge of medical office protocols/procedures
  7. Ability to plan, prioritize and complete delegated task with attention to detail and effective time management
  8. Ability to work well under pressure and with various time constraints is essential
  9. Ability to learn and retain information regarding patient care procedures
  10. Familiarity with basic computer operations, including but not limited to: MS Word- Excel-Outlook-PowerPoint
  11. Ability to work successfully with indirect supervision and autonomy
  12. Ability to work with culturally diverse people
  13. Typical work hours are from 8:00am to 5:00pm. Attendance at evening and/or weekend meetings and events may be required as necessary, with potential need to work one evening per week.
  14. Must have valid driver’s license and personal transportation to travel between office/county locations.
  15. Neat, well-groomed and professional appearance


Education: Associate’s degree in a health-related or social service field or other discipline related to the field; or equivalent combination of training and experience. All degrees must be received from appropriately accredited institutions.


Experience Required: Minimum 1-year experience working within clinical setting (public/community health) and/or Medical Assistant certification

Key Responsibilities (10-Core)

  1. Demonstrate and apply knowledge of medical conditions, including but not limited to medical terminology, transmission, risk reduction, disease referral resources and accessible treatment programs.
  2. Update/maintain accurate and secure patient medical records.
  3. Assist with the integration of policies and protocols related to HIV and HIV prevention, sexual health, reproductive health, general health promotion and community health, etc.
  4. Coordinate/facilitate referrals in order to link patients to available programs/care/resources, based on participant eligibility by scheduling appointments and removing barriers to care, while tracking all linkage activities.
  5. Perform community outreach to promote program services, while collaborating with community partners. Includes but not limited to attending community events, coordinating community meetings, providing educational presentations, etc.
  6. Assist with the completion of programmatic reports.
  7. Actively participates in QI activities, in order to evaluate program activities to improve quality of care and program efficiencies.
  8. Assist with completing projects and tasks for current programs and/or developing initiatives.
  9. Participates in staff/committee/agency meetings as required
  10. Performs other clinical or administrative functions as directed by the supervisor.
