Job description

CareHarmony works comprehensively with providers to deliver value-based care management initiatives for their patients. Our team is driven by their passion for the level of care delivered; experts in assisting patients and caregivers to navigate a sometimes-fractured healthcare system and consistently prioritize a high-quality standard so patients may better manage chronic illnesses and achieve success.

Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs.

Requirements

Responsibilities/Day in the Life:

  • Receive new patient referrals from clients
  • Create charts for all new patients
  • Input medical information in new chart
  • Help Care Coordinators with patient needs as applicable
  • Foster relationships with their team to ensure the CareHarmony culture is positive and supportive.
  • Other duties as applies

Required Skills/Abilities:

  • Great attention to detail

  • Excellent verbal and communication skills
  • Excellent professional presence when dealing with colleagues
  • Excellent time management skills with a proven ability to meet critical deadlines.
  • Fundamental Skills with Microsoft Office Suite

Required Education and Experience:

  • Medical Assistant certificate must be active and in good standing

Physical Requirements

  • This position is sedentary and will require sitting for long periods of time
  • This position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time 
  • The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations
