Job description

Integra Vascular provides patient-focused, preventative vascular care to the people of our communities and to long-term residents of medical facilities nationwide. Our expert physicians create cutting-edge treatment plans that best serve the needs of each individual and improve their quality of life.

Responsibilities:

  • Performs on-site screening clinical assessments to determine appropriate patient treatment plans of residents at client facilities by the completion of a comprehensive assessment

  • Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients

  • Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs

  • Assists with the provision of care in accordance with facility, state, and federal regulations

  • Monitors the effectiveness of interventions

  • Maintains relationships with all appropriate contacts in facilities

  • Educates family members, clients, and prospective accounts regarding services and programs

  • Assists with the development and maintenance of physician relations, as assigned

  • Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner

  • Other Duties as assigned by Medical Director

Qualifications:

  • Current Nurse Practitioner/ Physician Assistant License

  • Degree from an accredited Nurse Practitioner/ Physician Assistant Program

  • ACLS

  • Current infection control certification

  • Strong clinical collaboration with physicians

  • Proficient computer skills

  • PACS and EMR experience preferred

  • Committed to professional growth and learning

  • Clean driving record

  • Personally owned vehicle required

  • Local travel required

  • Not listed on the OIG exclusion list

Preferred Experience:

  • Vascular Care: Venous Insufficiency, Peripheral Arterial Disease

  • Image Guided Injectables

  • Advanced Geriatric

  • Skilled Nursing Facilities, Assisted Living Facilities
