Hill Country Community Clinic is a community-based, patient-centered Federally Qualified Health Center (FQHC) and Certified Community Behavioral Health Clinic (CCBHC) dedicated to providing high-quality, comprehensive, and affordable healthcare services to underserved populations in Shasta County, CA. We are committed to reducing health disparities and improving the well-being of our diverse community.

Position Overview

We are seeking a compassionate and dedicated Physician to join our team. The ideal candidate will provide high-quality, patient-centered primary care services to individuals of all ages, ensuring comprehensive and integrated healthcare. This role is vital in fulfilling our mission to serve medically underserved populations and improve health outcomes in the community.

Responsibilities
  • Provide direct patient care, including preventive care, diagnosis, treatment, and management of acute and chronic conditions.
  • Develop and implement individualized treatment plans and educate patients on health maintenance and disease prevention.
  • Collaborate with a multidisciplinary team, including nurses, behavioral health providers, case managers, and care coordinators to coordinate patient care.
  • Maintain accurate and up-to-date electronic health records (EHR) in compliance with federal, state, and organizational standards.
  • Participate in quality improvement initiatives, peer reviews, and compliance with FQHC guidelines and best practices.
  • Provide culturally competent care tailored to the needs of a diverse patient population.
  • Supervise and mentor advanced practice providers (e.g., nurse practitioners, physician assistants) as needed.
  • Support community outreach, health education, and patient engagement initiatives.
  • Adhere to all FQHC regulatory requirements, including HRSA, HIPAA, and OSHA guidelines.
Qualifications
  • Medical degree (MD or DO) from an accredited institution.
  • Board certification or board eligibility in Family Medicine, Internal Medicine, or Pediatrics.
  • Valid and unrestricted California medical license.
