POSITION SUMMARY
• The primary purpose of this position is to assist in planning, implementing, and evaluating social service programs in accordance with federal, state, and local standards, as well as the established policies and procedures of Bethany Lutheran Home. Furthermore, this position must ensure that the psychological and social needs of the residents are met on an individual basis at all times.
GENERAL JOB DUTIES
• All employees are responsible for knowing the employee handbook, along with the mission and vision of the Facility, and must conduct themselves with behavior that supports these values.
• All employees understand the safety and well-being of both the residents and staff members is the number one priority of their Facility.
• All employees are responsible for making the most of their working time and reach maximum potential by being on time for appointments, checking voicemail/emails as appropriate and completing deadlines in a timely manner.
• All employees are expected to escalate issues as necessary.
• You may be required to attend training sessions.
• Other projects and/or duties as assigned.
SOCIAL SERVICES FUNCTIONS:
• Meet with administration, nursing and other department heads to promote and plan social services, as directed by the Assistant Administrator.
• Assist in the development and implementation of our written policies and procedures that govern the operation of the facility.
• Interview/meet with residents and families on a regular basis to make sure their needs are being met and as needed basis when things arise.
• Coordinate and perform guided tours for prospective residents.
• Participate in pre-admission assessments which can be on paper or going off sight to their location.
• Responsible for completing accurate MDSs and UDAs.
• Coordinate care plan meetings, participating and making sure all documentation is complete and accurate.
• Submit and maintain accurate PASSRs as well as the coinciding care plans.
• Facilitate good communication with resident family members through family support services.
• Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment throughout the facility.
• Maintain and update referrals.
• Coordinate in-house room transfers along with accurate documentation.
• Represent the facility in dealings with outside agencies, including governmental agencies and third-party payers, or provide as an authorized representative of the facility when assigned by administration.
• Participate in quality improvement assurance, IDT, Medicare and other meetings as assigned.
• Prepare facility reports for Management Team and other reports as directed by the Administrator.
• Complete discharge planning and follow up once resident has returned to prior living arrangements.
• Maintain an adequate liaison with families and residents.
• Maintain grievance logs and make sure they are completed and submitted timely.
• Review and develop a plan of correction for deficiencies noted during survey inspections and provide a written copy of such plan to the Administrator for his/her review/approval. Review same with ombudsman personnel, as required.
• Accurately complete monthly ombudsman report.
• Maintain productive working relationship with the medical profession and other health-related facilities and organizations.
EDUCATION & EXPERIENCE REQUIREMENTS
• Must possess, a Bachelor’s degree in a human service field or greater, or qualifying experience.
PHYSICAL REQUIREMENTS
• Must be able to work in high-pressure or emergency situations that may face strict deadlines. Extended shifts may be necessary, and hours may exceed 40 per week to perform duties effectively, including some weekends and evenings.
• Ability to lift up to 50+ pounds or more with assistance. This position requires regular movements such as grasping, kneeling, lifting, pulling, pushing, reaching, repetitive movement, standing, and stooping. An individual in this position will be expected to use, assist, and demonstrate devices for handling or lifting residents.
• An individual in this position may be exposed to inside and outside environmental conditions, physical, chemical, or respiratory hazards.
• An individual in this position may be exposed to Bloodborne Pathogens.
