Title: Field Supervisor (CNA)

Reports to: Executive Administrator
Location: Irvine & Mission Viejo, CA
Schedule: Friday through Tuesday, Off on Wednesday and Thursday

POSITION SUMMARY
The Field Supervisor (CNA) will play a vital role in providing compassionate care and support to residents in their daily living activities. This full-time position requires flexibility, strong communication skills, and a proactive attitude to collaborate effectively with residents, caregivers, and administrators across multiple locations. We're seeking a self-motivated, energetic, and empathetic individual to foster a positive and supportive environment for our residents.
PRIMARY RESPONSIBILITIES
  • Check on residents daily - Create a schedule of facility rounds based on the needs of the residents.
  • Be able to collaborate with colleagues on the field and share responsibilities.
  • Assist in resident discharges - Remove resident files, medications, and DME from the home.
  • Observe residents for signs of infection (respiratory and urine)
  • Observe residents for skin tears and wounds.
  • Observe other changes in condition and report to Administrator.
  • Be able to follow protocols for calling 911.
  • Identify new resident care needs and communicate to Administrator; communicate the plan to meet needs to the caregivers.
  • Assist with activities of daily living.
  • Ensure resident vitals are taken daily.
  • Monitor resident weight gain or weight loss.
  • Audit resident medications to ensure correct medications are delivered from the pharmacy.
  • Collect lab specimens.
  • Report resident, caregiver, family, and other issues to the Administrator/supervisor.
  • Engage in social interactions with residents in a friendly and positive demeanor.
  • Monitor and ensure caregivers are effectively performing their job duties.
  • Assist caregivers in keeping the homes, resident rooms, and restrooms clean and organized.

QUALIFICATIONS
  • High school diploma or equivalent required.
  • Active CNA Certification required
  • Criminal background clearance is required.
  • Two years of experience (assisted living setting preferred).
  • Knowledge of state elderly care regulations and procedures is highly valued.
  • Reliable transportation.
  • First Aid/CPR Certified.
  • Strong organizational, prioritizing, and analytical skills.
  • Pre-employment physical and TB Test required per state regulations.
  • Ability to read, write, and follow oral and written directions.
