Job description

We areย seeking a compassionate and organized Social Services Assistant to support the Social Services department in providing exceptional care and advocacy for residents. Working closely with the Social Services Director and interdisciplinary team, this role helps address residents' psychosocial needs, assists with admissions and discharge planning, and promotes a positive resident and family experience while ensuring compliance with applicable regulations.ย 

Key Responsibilitiesย 

  • Assist the Social Services Director in meeting the emotional, social, and psychosocial needs of residents. ย 

  • Complete social histories, assessments, and required documentation as assigned. ย 

  • Support residents and families through the admission, adjustment, and discharge processes. ย 

  • Assist with discharge planning and coordinate community resources and post-discharge services. ย 

  • Provide information and support to residents, families, and responsible parties regarding available services and resources. ย 

  • Participate in interdisciplinary care plan meetings and collaborate with Nursing, Therapy, Admissions, and other departments. ย 

  • Help advocate for resident rights and address resident concerns or grievances. ย 

  • Maintain accurate and timely documentation in accordance with facility policies and regulatory requirements. ย 

  • Assist with resident support groups, family meetings, and other social service programs as needed. ย 

  • Help coordinate referrals to outside agencies and community resources. ย 

  • Maintain resident confidentiality and comply with HIPAA and all applicable federal and state regulations. ย 

  • Perform other duties as assigned by the Social Services Director or Administrator. ย

Qualificationsย 

  • High school diploma or equivalent required; Associate's or Bachelor's degree in Social Work, Human Services, Psychology, or a related field preferred. ย 

  • Previous experience in social services, healthcare, long-term care, or human services preferred. ย 

  • Knowledge of resident rights, discharge planning, and community resources is preferred. ย 

  • Strong interpersonal, communication, and organizational skills. ย 

  • Ability to work effectively with residents, families, and an interdisciplinary team. ย 

  • Proficiency with electronic health records and Microsoft Office preferred. ย
