Braille Clinical Services is seeking dedicated and compassionate Licensed Marriage and Family Therapists (LMFT) to join our team in Irvine, California. • Assist patients and families by utilizing appropriate community resources to promote optimal functioning throughout their care.
• Educate patients and families about discharge options based on mental health status, legal considerations, and insurance coverage, ensuring informed decision-making.
• Provide continuity of care through comprehensive referrals to community services and support systems, assuring that services remain accessible post-discharge.
• Proactively schedule follow-up appointments with psychiatric providers within one week following discharge and share relevant discharge information to facilitate smooth transitions.
• Conduct thorough safety planning with patients and families, which may include measures such as gun removal in high-risk situations, to enhance patient safety and reduce potential harm.
• Maintain open and effective communication with interdisciplinary healthcare teams, court officers, conservators’ offices, and patient advocates to promote patient progress and maintain patient rights.

• Must possess a current and active state license as a Licensed Marriage and Family Therapist (LMFT).

• At least two years of relevant experience in providing therapeutic services to individuals and families is essential.

• Experience working within outpatient settings or similar environments.
• Familiarity with electronic health record (EHR) systems and telehealth practices.
• Previous involvement in coordinating care with multidisciplinary teams.

The annual salary for this position ranges from $120,000 to $130,000, commensurate with experience and qualifications.
