Position Description:

The Clinic/Community Nurse will provide assistance to clients within the clinic programs, substance use programming as well as support the Assertive Community Treatment team and IRSH (Intensive Residential Support Home) programs.

The Clinic/Community Nurse will assess and monitor the client’s on-going medical, psychiatric, and social needs. This position will collaborate with internal and external providers to coordinate health care treatment as needed for the clients in all programs. This position will also participate in assessment and development of service planning for the ACT team to identify and implement supports needed.

This position does require travel within Black Hawk County for the ACT program and to the IRSH homes located in Waterloo, Ft Dodge, Newton, Marshalltown and Indianola and the coverage areas for the ACT team. The Clinic/Community Nurse will be available to help the Nurse Practitioners with medication questions, rooming clients when they are in the clinic for appointments and facilitating conversations, refills with pharmacies as needed. This position may need to respond to crisis calls during the workday if needed.

Shift: 1st shift Monday-Friday 8:00am-5:00pm

Job Description:

  • Work directly with program participants to assess physical health care needs and coordinate with member’s health care professionals on those needs.

  • Ensuring care given to those with complex mental health needs is appropriate and coordinated, incorporating best practice for treatment, risk assessment and multi-agency service coordination.

  • Act as the team expert on medical issues of the members.

  • Need to use patient registries, population management, gaps in care analysis tools in serving members.

  • Responsible for administering initial TB tests for staff, and completing all annual follow up screenings.

  • Oversight and tracking of all incident reports

  • Communicate and coordinate with primary care and specialty care providers to review and plan care for members.

  • Participate in assessment and development of service plans along with Assist in coordination of medically related service needs and supports for all programs.

  • Understands self-management tools, usage and need and can develop/organize for the population being served.

  • Follow and enforce standards of both ACT and IRSH programming.

  • Conduct Quality Assurance reviews for ACT and IRSH.

  • Provide assistance and support for staff with medication administration for those in IRSH home.

  • Implementing preventive, as well as remedial, health and wellness plans.

  • Cross-training of other team members in use of health and wellness interventions as individually indicated.

  • Work closely with other members of the team and staff from other key agencies.

  • Provide education and training to agency staff regarding health issues.

  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.

  • Ensures development and implementation of transition/discharge or after care plans as needed.

  • Abides by all specific program and Agency procedures, policies, and requirements.

  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.

  • Supervise the provision of medication monitoring/ administration by staff including supervision and providing training.

  • Complete nursing assessments and medication reconciliation for clients.

  • Provide medical and psychiatric education to clients and their families.

  • Assist with resources/benefit acquisition and coordination as needed for ACT team.

  • Conducts medical/health/wellness assessments to identify health care needs and assist in developing health care goals with the treatment team.

  • Track required healthcare assessment and screenings.

  • Having knowledge of physical health aspects of client care, including weight, nutrition, medication education, assessing and education of side effects, risks and benefits of medication

Qualifications:

  • Active RN license for the State of Iowa Required.

  • RN with BS is preferred.

  • Minimum of 2 years nursing practice with those with complex mental health/substance use.
