Client Details: City Lawrenceville State GA

Cancellation Policy: You are receiving this email because your agency may have staff that will receiving extension for the State of GA program with HWL. In order to get the extensions approved it was agreed that there will be a change to the 30-day cancellation notice provision for those extensions due to the shortened extension durations which are 4-6 weeks. You will see the language below reflected in the confirmation agreements being processed by the HWL team. Please make sure that your staff are aware of this change as you confirm extensions on your end. Customer may cancel a Provider's assignment without cause at any time upon providing notice thereof. If Customer cancels a Provider's assignment extension less than fourteen (14) day prior to the scheduled commencement of such assignment extension by a fully credentialed Provider, Agency may invoice Customer for thirty-six (36) hours at the proposed assignment weekday bill rate, and to the extent substantiated by written documentation, for actual fees and charges that may result from cancellation of an Assignment, including but not limited to lost rents up to a maximum of one month, security deposits and airfare ("Costs"). After a Provider has begun working on an assignment extension, Customer may terminate the assignment without any penalty if a fourteen (14) day advance notice is provided to Agency. If less than a fourteen (14) day advance notice is given by Customer, then Customer will pay for the lesser of (i) the remainder on the assignment extension, or (ii) Agency may invoice Customer for one hundred forty-four (144) hours at the proposed assignment weekday bill rate.
