What You'll Do
  • Complete comprehensive speech, language, cognitive, and swallowing evaluations
  • Develop individualized treatment plans based on patient goals and clinical needs
  • Provide skilled speech therapy services in patients' homes
  • Educate patients and caregivers on communication techniques, cognitive strategies, and safe swallowing practices
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with physicians and the interdisciplinary care team
  • Complete timely and accurate clinical documentation using WellSky

Qualifications
  • Active Washington, DC Speech-Language Pathologist License Required
  • Certificate of Clinical Competence (CCC-SLP) Preferred
  • Valid Driver's License and Reliable Transportation
  • Strong Clinical Assessment, Communication, and Documentation Skills
  • Home Health Experience Preferred but Not Required
  • OASIS Experience is a Plus
  • Passion for Delivering Exceptional Patient-Centered Care
