How you’ll make an impact in this role
  • Provide comprehensive pharmaceutical care for the PACE (Program of All-Inclusive Care for the Elderly) population, supporting adults 55+ in maintaining independence and aging safely.
  • Review medication orders to identify, prevent, and resolve drug-related problems while recommending appropriate formulary alternatives to optimize patient outcomes.
  • Accurately compound and dispense medications, ensuring safe and timely delivery for a medically complex geriatric population.
  • Educate and counsel patients, caregivers, and interdisciplinary healthcare teams on medication therapies, adherence, and disease management.
  • Collaborate with physicians and care teams while supervising pharmacy support staff and assisting with training/precepting pharmacy students as needed

What minimum requirements you’ll need

Licensure / Certification / Registration:

  • Pharmacist obtained prior to hire date or job transfer date required.

Education:

  • Bachelor's or advanced degree in Pharmacy required.

What additional preferences we're seeking
  • Previous pharmacist experience in a PACE (Program of All-Inclusive Care for the Elderly) setting preferred.
  • Experience working with adults aged 55+ and geriatric populations strongly preferred.
  • Prior long-term care (LTC) pharmacy experience preferred.
