Job description

Nurse Practice Manager
Location: Rochester, NY
Reports to: VP, Operations
Company: Town Square Health
About Town Square Health
Town Square Health is reimagining what healthcare can be. We're building a first-of-its-kind value-based care model with a singular mission: to set the gold standard for how Americans experience healthcare. Our multi-specialty health centers serve Medicare-eligible patients, and we're growing fast, with expansion into multiple markets on the horizon. If you're bold, collaborative, and driven to make a real difference in people's lives, we'd love to have you on our team!
The Opportunity
This is not a traditional practice management role.
You'll join at an early stage with the chance to help build a health center and a care model from the ground up. That means real ownership, real influence, and the ability to shape how clinical and operational excellence comes to life day-to-day.
There is also real complexity. Building a patient panel, establishing workflows, and creating a high-functioning team takes time, problem-solving, and resilience. For the right person, that's exactly what makes this exciting.
You'll have the opportunity to:
  • Build and lead a care team in a brand-new environment
  • Influence how care is delivered and coordinated
  • Improve outcomes for Medicare patients through a more thoughtful, proactive approach
  • Help define what "great" looks like as we scale into new markets
What You'll Do
You'll be at the intersection of clinical excellence and operational leadership, blending direct patient care with the strategic oversight needed to scale a growing practice. This isn't just about management; it's about providing real-time clinical guidance while simultaneously building the workflows and team structures that define our care model. You'll move fluidly between mentoring your staff and solving complex operational hurdles, owning outcomes from end to end. Beyond the day-to-day readiness of the clinic, you will be the primary architect of your team: recruiting, onboarding, and coaching a high-performing group to ensure safety, compliance, and a seamless experience for every patient.Key Responsibilities
Nurse Leadership
  • Provide direct patient care, triage, and education to ensure adherence to clinical standards and best practices.
  • Oversee and mentor clinical staff in workflows, patient safety, and quality care delivery utilizing a nursing lens.
Operational and Business Management
  • Lead day-to-day clinic operations, including scheduling and resource management, ensuring efficiency and compliance.
  • Achieve goals related to quality, financial performance, patient satisfaction, and staff engagement.
Team Engagement and Management
  • Lead recruitment, onboarding, training, supervision, and mentorship for a growing clinical team.
  • Act as an advocate for both patients and staff, fostering a supportive, inclusive, and patient-centered culture.
Who You Are
You're a builder. You're energized by starting something new and making it work. You combine strong clinical instincts with operational savvy and aren't afraid to roll up your sleeves to solve problems in real time. You lead with clarity, accountability, and empathy, and you know how to bring a team along with you.
Most importantly, you're driven by the belief that healthcare can be better, and you want to be part of the team proving it.
Required qualifications
  • 3+ years of progressive experience as a registered nurse in a primary care, community health, or ambulatory setting.
  • Current Registered Nurse (RN) license in New York.
  • Bachelor of Science in Nursing (BSN) required; Master's degree in Nursing, Business, Healthcare Administration, or related field preferred.
Skills and Knowledge
  • Deep understanding of clinical judgment, patient care, and problem-solving skills.
  • Experience with healthcare operations, clinical practices, billing, reimbursement, and quality improvement.
  • Experience supervising, mentoring, and providing performance management to direct reports.
  • Strong analytical skills and comfort with data-driven decision-making.
  • Excellent leadership, collaboration, and communication abilities.
  • Mission-driven, collaborative mindset and a passion for improving access to high-quality primary care.

What We Offer
Starting salary range of $90,000-105,000/year, based on a full-time, 40-hour work week
  • Consistent schedule
  • Comprehensive benefits package (medical, dental, vision, 401K)
  • 25 paid vacation days and 8 paid holidays
  • Opportunity to build something from the ground up
  • Direct impact on patient care and outcomes
  • High visibility and influence in a growing organization
  • Collaborative, mission-driven team
