Job Summary:ย 

The official job title is Lead Care Manager (LCM).ย 

The Lead Care Manager (LCM) role involves developing personalized care plans, coordinatingย member services, and collaborating closely with members and families, as well as Primary Care Providersย to ensure they receiveย necessary medical treatment and support. The LCM will consult with members toย determineย their needs, develop individualized action plans, and work with care teams to manage theย member experience effectively. Providing emotional support, resolving administrative issues,ย and ensuringย timelyย access to care are key aspects of the position. Compassion, healthcareย knowledge, and exceptional customer service are essential qualities forย assistingย members inย becoming self-sufficient in health.ย ย The LCM will work with aย diverseย population of members enrolled in the Enhanced Care Management program, which may involve one or multiple members from the population ofย focusย section below.ย ย 

Responsibilities:ย 

  • Interview members to assess medical andย socialย determinantย of healthcare gapsย and provide education about their condition and medication, while developing individualized care plans.ย 
  • Respond to member inquiries and concerns, ensuring adherence to hospital and legal requirements.ย 
  • Collaborate with interdisciplinary teams,ย locateย medical and social resources, and coordinate social service plans.ย 
  • Maintainย on-goingย contact withย members,ย via telehealth and in-person visitation.ย 
  • Advocate for members, consult with healthcare providers, arrange appointments and treatment plans, evaluate member progress, andย assistย with healthcare barriers.ย 
  • Maintain empathy and professionalism while contacting members and families.ย 
  • Supporting behavioral health coordination, Substance Abuse and Community Resources.ย 
  • Performย additionalย duties as assigned.ย 

Populations of Focus:ย 

  • Individuals experiencing homelessness:ย Lacking a fixed, regular, and adequate nighttime residence.ย 
  • Individuals at risk for avoidable hospital or emergency departmentย utilization:ย Five or more emergency room visits in a six-month period that could have been avoided withย appropriate outpatientย care or improved treatment adherence.ย 
  • Individuals with Serious Mental Health and/ or substance use disorder needs:ย Adults who meet the eligibility criteria for participationย in, orย obtaining services through Specialist mental health services or the Drugย Medi-Calย organized delivery system orย Drugย Medi-Calย program.ย ย 
  • Individuals transitioning from incarceration/Justice Involved:ย Adults transitioning from a correctional setting or transitioned from a correction setting within the past 12 months, or children and youth who are transitioning from a youthย correctional facilityย or transitioned from being in a youthย correctional facilityย within the past 12 months.ย ย 
  • Adults living in the community and at risk for long-term care institutionalization:ย Adults who are living in the community who meet the SNF Level of Care criteria; or whoย requireย lower-acuity skilled nursingย or equipment for prevention, diagnosis, or treatment of acute illness or injury.ย 
  • Adult nursing facility residents transitioning to the community:ย Adult nursing residents who are interested in moving out of the institution, and areย likely candidatesย to do so successfully, andย are able toย resideย continuously in the community.ย 
  • Children and youth enrolled in California Children's Services (CCS) or CCS Whole Child Model (WCM) with Additional Needs Beyond the CCS condition:ย Children and youth enrolled in CCS or CCS WCM and are experiencing at least one complex social factor influencing their health.ย ย 
  • Children and Youth Involved in Child Welfareย 
  • Individuals with Intellectual/ Developmental Disabilities:ย Adults who have a diagnosed I/ DDย and qualify for eligibility in any other adult ECM population of focus.ย 
  • Pregnant and Postpartum individuals; Birth Equity Population of Focus:ย Adults and youth who are pregnant or postpartum and qualify for eligibility in any other adult or youthย ECM POF, orย are subject to racial and ethnic disparities.ย 

Education and Experience:ย 

  • High school diploma or GEDย required.ย 
  • Minimum of 1 year experience in case management, member care, customerย 
    service, call center, or member careย required.ย 
  • Valid California driver's license and valid vehicle insuranceย required.ย 
  • MA certificate or medical terminology knowledge preferred.ย 

Required Skills/Abilities:ย ย 

  • Bilingual in Arabic and English required.
  • Excellent communication, interpersonal, customer service and organizational skills.ย 
  • Computer skills for documentation,ย emailย and chat support.ย 
  • Proficient skills in working independently and collaboratively in a team to provide member care.ย 
  • Proficiencyย in multitasking, organization, and attention to detail isย requiredย for effectively providing care to multiple members simultaneously.ย 
  • Candidates shouldย exhibitย the capability toย utilizeย resources effectively for problem-solving whileย maintainingย composure under pressure in a compassionate manner.ย 
  • Applicants mustย demonstrateย proficient speaking, reading, and typing abilities andย possessย strong proofreading skills.ย 

Physical Requirements:ย ย 

  • Must be able to travel using personalย vehicleย to complete outreach visits. Mileage reimbursement for the use of your vehicle is at a standard rate.ย 
  • Must successfully complete andย maintainย current Tuberculosis (TB) testing and clearanceย in accordance withย company policy and applicable local/state health regulations as a condition of employment.ย 
  • Prolonged periods of sitting at an office desk on the computer.ย 
  • Lifting: Able toย lift upย to 15lbs.ย
