The Paraprofessional provides direct or general care to mental health patients from admission through discharge under the direction of a supervisory Qualified Professional (QP) and within the parameters of the state licensing board.   • Assist mental health patient(s) with management of resident’s care plan, scheduling ofservices, medication
  • Provide direct and indirect patient care services including personal care, meal prep,basic hygiene, lifting, dietary support, other personal needs and medical necessities in agroup home centered environment
  • Complete patient specific Paraprofessional documentation in the patient’s record relatedto daily care, medications, behavior, crisis incidents, communication with others, etc.
  • Report on potential compliance problems as outlined in the Employee Handbook
  • Model ethical and appropriate behavior, professional standards, compliance, andpersonal integrity
  • Must be able to perform the essential functions of the position with or withoutaccommodation
  • Clearly and concisely communicate the needs of residents and family, changes inresidents’ clinical condition, shift notes, and other issues
