CLIENT 1 — SELINSGROVE (AFTERNOON SUPPORT)
Weekdays — Afternoon

Care Needs:
  • Assistance with ADLs
  • Light housekeeping
  • Meal preparation
  • Walking support to help him regain strength
  • Mobility assistance
  • Client’s left arm is amputated, making daily tasks difficult
  • Transportation for errands and appointments
  • Driver required
