Located in Danville, Virginia, we serve patients of all ages who reside in Danville and Pittsylvania County, and in parts of Halifax and Henry Counties. Our services include skilled nursing, physical therapy, occupational therapy, speech-language pathology, and home health aide.
Job Description
All Care Home Health is currently seeking a motivated Physical Therapist and/or Physical Therapist Assistant to deliver exemplary care and enthusiasm in working with patients of all ages.
We currently have openings for Physical Therapists and Physical Therapy Assistants in the Danville, Virginia area. COME JOIN OUR TEAM!
These positions are PRN, meaning you have huge flexibility to work as little or as often as you want. These positions require travel among patient homes in Danville and Pittsylvania County, Virginia. You must provide your own transportation.
JOB DESCRIPTION
The Physical Therapist in Home Health is responsible for the assessment of patient care needs, evaluation of level of function, development of the treatment plan (revising as necessary), preparation of clinical and progress notes, advising consultation of family and other agency personnel, and participation in in-service programs. The Physical Therapy Assistant will support the Physical Therapist in treating the patient, communicating with the primary therapist, and all documentation as necessary.
A candidate must:
  • Assist the physician in evaluating the level of function, disability, injury, or disease process of individual patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
  • Provide treatment as ordered by the attending physician to relieve pain. Develop or restore function and maintain maximum performance using therapy modalities.
  • Observe, record, and report the patient's reaction to the therapy program or any changes in the patient's condition.
  • Interpret, plan, establish, and exercise proper treatment program in accordance with the physician's referral, periodically reviewing the patient's program and response and making recommendations to the physician relative to the need of continuing or discontinuing the physical therapy treatment.
  • Instruct, teach, and supervise patients, their families, and other health team personnel regarding physical therapy procedures as related to the patient's therapy program.
  • Evaluate the home environment and make appropriate recommendations.
  • Document each visit to the patient and incorporate notes into the clinical record

Qualifications
  • License to practice as a Physical Therapist or Physical Therapist Assistant in the Commonwealth of Virginia
  • Current CPR certification
  • Valid Driver's License
  • No Criminal History
  • Experience in Home Health preferred, not required
  • Must have at least 1 year experience as a practicing PT or PTA
