This role requires a presence in the homes of our patients, ensuring a comfortable and personalized setting for therapy. The home care environment allows for tailored treatment plans that can adapt to each individual's unique living situation. While this role is largely autonomous, you will be supported by a robust team of professionals, including administrative staff and fellow therapists, to ensure optimal treatment outcomes and an excellent patient experience.

• Conduct thorough consultations with patients and their families to assess physical conditions, symptoms, and personal goals.
• Utilize clinical skills to diagnose movement dysfunctions, establishing individualized and effective treatment plans that incorporate therapeutic exercises, manual techniques, and modalities.
• Educate patients about therapeutic exercise techniques, ensuring they understand the purpose and method to enhance their recovery at home.
• Implement restorative treatments, such as massage or myofascial release, to promote healing and enhance mobility.
• Assist patients in utilizing adaptive equipment such as wheelchairs and walkers, ensuring safety and independence.
• Maintain accurate and up-to-date patient records, tracking progress towards therapeutic goals and adapting treatment strategies as necessary.
• Provide ongoing education and guidance to patients and their families on in-home treatment options and exercises to facilitate continued recovery.

You will work with a diverse population, including children recovering from sports injuries, adults managing chronic pain, and elderly patients seeking rehabilitation after surgery. Each patient presents unique needs, requiring innovative and adaptive approaches to therapy. Your contributions will have a significant impact on their rehabilitation journey, aiding not only in physical recovery but also in emotional and social reintegration.

• A Bachelor's Degree in Physical Therapy, demonstrating a strong foundational knowledge of treatment modalities and patient care.
• A valid state license in Physical Therapy to practice in Florida, ensuring adherence to legal and professional standards.
• Excellent communication skills, both written and verbal, which are crucial for effective patient interactions and documentation.
• Proficient computer skills, including familiarity with electronic health record (EHR) systems for efficient record-keeping and care coordination.
• Flexibility and agility in adapting treatment plans and approaches as required by patient needs and home environments.

Ideal candidates will possess a minimum of 2 years of experience in a clinical or home care setting, showcasing their ability to work independently and collaboratively within a multidisciplinary team. Experience in pediatric therapy or geriatric rehabilitation is highly valued, but we encourage applicants from all backgrounds of physical therapy.
