Job description

Nurse Practitioner Palliative Care

Job Summary and Responsibilities

This is a PRN position

CommonSpirit Health at Home leverages AI-powered clinical documentation to reduce documentation, enhance work-life balance, and maximize patient time.

As a Nurse Practitioner with CommonSpirit Health at Home, you'll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient's continuing eligibility for palliative care.

Responsibilities:

  • Provides care following the plan of treatment and patient care plan to assist in determination of palliative care eligibility.
  • Establishes a plan of care for continued needs based on the client assessment.
  • Reports changes in patient condition to appropriate personnel and the attending physician.
  • Provides patient care education, counseling, medication advice, instructions and follow-up to clients and other caregivers.
  • Participates in coordination of services with other disciplines and community resources as needed.
  • Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.
  • Assists with the development of educational programs in the palliative care program for diverse age groups.
  • All other duties as assigned.

Job Requirements

  • Must have full prescriptive authority
  • Graduate of an accredited school of nursing.
  • Currently licensed to practice in the state serviced, with Masters in nursing.
  • At least 1 year of nursing experience
  • Must obtain and maintain current CPR
  • Strong critical thinking, clinical expertise, case management, and a passion for patient care.
  • Valid driver's license and auto insurance coverage according to the CHI Health at Home policy.
