Care Coordinator Home Health

Primary Location: Phoenix, Arizona

Are you a clinician who thrives on building relationships, solving problems, and helping patients achieve the best possible outcomes? Use your clinical expertise to impact patients and families without the demands of direct patient care, productivity standards, weekend visits, or endless documentation. We are looking for a passionate clinician to join our team as a Care Coordinator, serving as a trusted clinical resource to hospitals, skilled nursing facilities, rehabilitation centers, physicians, and other healthcare partners throughout the community. This is not traditional sales, this is clinical leadership, patient advocacy, and relationship-building at its finest.

As a Care Coordinator, you become the clinical bridge between healthcare providers and home health services. You'll help ensure patients receive the right care, at the right time, in the right setting, allowing them to recover safely and successfully at home. Every relationship you build has the potential to improve outcomes for countless patients and families.

This position will focus on the Mayo Clinic & Hospital

  • Build meaningful relationships with hospitals, physicians, skilled nursing facilities, and community healthcare partners
  • Assist patients and families during critical transitions from hospital or post-acute settings into home health care
  • Serve as a trusted clinical resource for referral partners
  • Educate healthcare professionals on the benefits and services available through home health
  • Help remove barriers to care and improve continuity of care
  • Advocate for patients and ensure seamless care transitions
  • Collaborate with clinical and operational teams to support growth and exceptional patient experiences

Must be a graduate of an approved school of nursing or therapy.

  • Must be licensed in the state where they currently practice.
  • Must have two (2) years of demonstrated field experience.
  • Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.
  • Must have basic demonstrated technology skills, including operation of a mobile device.

  • Preferred Clinical Backgrounds
  • Registered Nurse (RN)
  • Licensed Practical Nurse (LPN)
  • Physical Therapist (PT)
  • Physical Therapist Assistant (PTA)
  • Occupational Therapist (OT)
  • Certified Occupational Therapy Assistant (COTA)
  • Three (3) years of field experience is preferred.
  • Previous experience in home health or healthcare sales is preferred.

Requirements

  • Must possess a valid state driver license
  • Must maintain automobile liability insurance as required by law
  • Must maintain dependable transportation in good working condition
  • Must be able to safely drive an automobile in all types of weather conditions
