Job Description Summary:
The Spira Care Advanced Practice Provider (APP) provides advanced primary care in partnership with Blue Cross and Blue Shield of Kansas City (Blue KC) to deliver best-in-class primary care to individuals and populations in Kansas City and surrounding communities. At Spira Care, we value high-value care and whole-person care. Clinicians work at "top of license" and collaborate with physicians, APPs, behavioral health consultants, diabetes care specialists, clinical pharmacists, and nursing professionals supported by care managers and referral specialists to provide evidence based medical care, integrated behavioral health, and attention to unmet complex needs. We value quality, affordability, patient experience, care team well-being and health equity.
Job Description:
  • Work with the healthcare team, exercising independent medical judgment within the scope of licensure to provide with advanced primary care consistent with best practices and Spira Care policy.
  • Unless otherwise agreed to by leadership, provide patient care at least ninety percent of the time (patient care is provided virtually or in-person at primary care center AND sister care centers, when needed).
  • Provide rotating on-call coverage for nights and weekends as assigned.
  • Appropriate engagement with collaborating physician(s) in alignment with policy and/or best practices.
  • Clear, complete, and timely clinical documentation in EMR per risk management and Spira Care policy.
  • Participation in activities aligned to reducing wasteful spending in healthcare via judicious clinical decision-making.
  • Leading team-based care initiatives (ex: team huddles).
  • Adhere to departmental policies and procedures including environmental and infection control standards.
  • Maintain patient confidentiality and comply with all federal and state health information privacy laws.
  • Participate in developing clinical initiatives, interventions, and continuous quality improvement activities + partnering with team members from business and health plan functional units in so doing.
  • Consultation with and adherence to Blue KC Medical Policies re: coverage and medical necessity + participation in key utilization management activities (ex: peer-to-peer consultations with health plan medical directors).
  • Consultation with teams and team members in care centers to optimize patient outcomes: behavioral health consultants, clinical pharmacists, diabetes care specialists, care managers, health coaches, practice facilitators, and others.
  • Execute and provide services consistent with a collaborative practice agreement or active practice request form and written agreement.

Minimum Qualifications
  • Successful completion of an accredited APP (NP and PA) training program.
  • BLS certification from American Red Cross or American Heart Association.
  • 1-2 years of experience in the primary care setting providing the full scope of services typically considered in scope for Family Medicine.
  • Current Kansas and Missouri License, or in process, at time of hire
  • Bureau of Narcotics and Dangerous Drugs registration required in Missouri.
  • Current Kansas and Missouri Drug Enforcement Agency (DEA) registration required upon hire or once clinical practice hours are completed.
  • Maintain licensures and certifications throughout employment
  • Experience with billing, coding and documentation practices to meet Medicare requirements.
  • Spira Care has specific vaccination requirements as a condition of employment. You must be able to comply with this condition of employment with or without reasonable accommodation based on a sincerely held religious belief or a disability.
  • Meet all requirements to provide services under Spira Care's provider agreement with Blue KC.

Preferred Qualifications
  • 3+ years' primary care experience
  • Experience with healthcare quality improvement initiatives.
