🌟 We're Hiring: Advanced Practice Provider - Palliative Care (Full-Time Days)

Join our compassionate and collaborative Palliative Care Team and make a meaningful difference in the lives of patients and their families.

Position Details
  • Full-Time Day Shift
  • Schedule: Monday-Friday, 8:00 AM - 5:00 PM
  • Primary Location: Saint Luke's Plaza
  • Opportunity to float to additional sites as needed
About the Role

As an Advanced Practice Provider (APP), you will provide advanced clinical care to assigned patient populations while serving as a key member of an interdisciplinary team. Responsibilities include:

  • Assessing, collecting, and analyzing patient health data
  • Developing, implementing, and evaluating individualized plans of care
  • Coordinating comprehensive patient care and case management services
  • Providing expert clinical consultation
  • Utilizing prescriptive authority, procedures, and treatments in accordance with state and federal regulations
  • Educating patients, families, and healthcare staff
  • Managing high-risk patient populations
  • Supporting quality improvement and research initiatives
  • Promoting positive patient outcomes and enhancing care delivery systems
Call, Weekend & Holiday Requirements
  • Call coverage: One week (7 consecutive days) at a time
  • Currently, call and weekend coverage are provided only at Saint Luke's Plaza
  • Call and weekend commitment: Approximately 1-2 times per quarter
  • Holiday commitment: One holiday per year
