What does an adoption case manager do?

An Adoption Case Manager is a social work professional who guides children and prospective adoptive families through the adoption process. Their responsibilities include assessing the needs of children, evaluating adoptive families, facilitating placement, and providing ongoing support to ensure successful adoptions. They also handle legal documentation, conduct home studies, and coordinate with social services and legal systems. The goal is to ensure the child's best interests are met and the adoption process runs smoothly for all parties involved.

How does an adoption case manager typically collaborate with families and other professionals during the adoption process?

Adoption Case Managers work closely with prospective adoptive families, birth parents, and a multidisciplinary team that may include social workers, therapists, and legal professionals. They facilitate communication between all parties, coordinate home studies and background checks, and ensure that each step of the adoption process complies with legal and agency requirements. Regular meetings and updates are common, and case managers often provide education, support, and resources to help families navigate challenges. This collaborative approach ensures the child's best interests are prioritized throughout the adoption journey.

What are the key skills and qualifications needed to thrive as an adoption case manager, and why are they important?

An Adoption Case Manager needs a background in social work or a related field, typically with a relevant degree and, in many cases, state certification or licensure. Familiarity with case management software, state adoption regulations, and child welfare systems is crucial. Outstanding interpersonal skills, empathy, and strong organizational abilities set top professionals apart in this role. These competencies ensure that children and families receive effective support, guidance, and advocacy throughout the adoption process.

Job description

Who We Are

Action for Children is the local child care resource and referral agency in Central Ohio. Our mission is to transform the lives of children by supporting, empowering, and advocating for the adults who make the biggest impact on children's livesโ€”their parents, caregivers, and teachers.

What You'll Do

Action for Children is seeking a dedicated and resourceful Case Manager to join our fatherhood services team. In this role, you will walk alongside program participants to identify barriers to responsible fatherhood and develop individualized plans to address them. Drawing on your professional experience and community knowledge, you will connect fathers with the resources, support, and encouragement they need to achieve their goals as parents and engaged members of their communities.

Major Responsibilities
  • Assess participants to determine barriers and challenges to responsible fatherhood.
  • Develop Individual Service Plans (ISPs) with participants and conduct regular follow-up meetings to review progress and provide guidance.
  • Connect participants with community resources related to housing, basic needs, education, legal services, child support, parenting time, and custody.
  • Maintain current knowledge of employment and workforce development resources, including job training, placement services, and employer partnerships.
  • Document all participant contacts, activities, and outcomes accurately and in real time.
  • Complete intake and exit processes, including all required surveys, assessments, and supporting documentation.
  • Build and maintain relationships with community partners and service providers throughout Central Ohio.

Please Note: This position requires weekly travel throughout Central Ohio and flexibility to work some evenings and weekends.

Successful Candidates Will Have
  • Bachelor's degree and demonstrated experience in social services, case management, or a related human services field.
  • Proven ability to maintain timely and accurate program records and data.
  • Knowledge of Central Ohio community resources and social service systems.
  • Ability to build rapport and work effectively with individuals from diverse backgrounds and cultures.
  • Proficiency with Microsoft Office Suite and other technology.
  • Valid driver's license, reliable personal vehicle, and proof of current auto insurance.

The ideal candidate values collaboration, diversity, growth, and respect for all people. They are highly motivated, creative, flexible, and committed to the success of both the program and the team. This position requires exceptional attention to detail, strong organizational and communication skills, and a proactive approach to problem-solving. The successful candidate will be comfortable engaging with male participants in a variety of settings, including correctional facilities, and will have a genuine passion for helping fathers become the parents and community members they aspire to be.

Desired but Not Required
  • Bilingual (English/Spanish)
Why You'll Love Working With Us
  • Comprehensive health, dental, and vision insurance beginning within 30 days of hire.
  • Generous paid time off, including vacation, personal days, and holidays.
  • Hybrid work schedule with a combination of remote and in-office work.
  • Retirement savings plan with employer match (up to 1%).
  • Twelve weeks of fully paid parental leave for birth, adoption, and foster care placements.
