Department: Nursing Administration

Summary:
The House Clinical Coordinator (HCC) is a registered nurse who is accountable for effective coordination of patient care needs. The HCC performs a variety of clinical and managerial duties within a hospital as assigned. The HCC coordinates care for patients, acting as a liaison between departments as well as between patients and their families and health care professionals. The HCC will assist with monitoring of patient care delivery and resolve any issues that arise related to patient safety, clinical outcomes, or patient experience as they are notified by the physician or staff. The HCC is an active member of the multidisciplinary team and collaborates in the development, execution and evaluation of the multidisciplinary plan of care. He/She considers factors related to ethics, safety, effectiveness and cost in planning and delivering care. The HCC establishes goals and strategies for meeting the continuing care needs of the patient, family, and/or other care provider. He/She ensures compliance with hospital policies, procedures, and quality standards. He/She provides care in collaboration with other nursing staff members when needed. The HCC demonstrates knowledge of and participates in improving organizational performance activities. The HCC acquires and maintains knowledge in nursing practice and assumes responsibility for the professional development of self, other nursing staff and students. The HCC works with the team to assure that systems of care are in place for patients to move across the continuum of care. This includes development of patient care protocols, order sets, and systems that allow care to take place in an effective and efficient manner. Responds to all emergency medical situations in house and provides assistance as needed. Also puts systems in place to evaluate the effectiveness of patient care.
