Program: Mental Health Group Home

Location: New Castle County

Status: Part-time

Schedule: TBD

ABOUT THE ROLE

The Certified Nursing Assistant (CNA) ensures the safety and supervision of individuals supported within the unit while fulfilling care plan obligations. The CNA assists with activities of daily living, healthcare coordination, and transportation, working closely with the Registered Nurse and Program Director to maintain a safe and healthy environment. Administrative tasks may also be assigned.


RESPONSIBILITIES

  • Administer medications and prescribed therapies safely and accurately.
  • Provide personal care (bathing, grooming, diapering, etc.) to maintain health and appearance.
  • Engage in physical activity to meet Crisis Prevention and Intervention requirements.
  • Implement supportive interventions during crises (e.g., aggression or self-injury).
  • Lift and transfer individuals as needed based on ability level.
  • Accompany and support individuals during community activities.
  • Provide services in various settings (home, community, hospitals, day programs).
  • Maintain cleanliness and safety in the home; report site issues promptly.
  • Document ISP outcomes and participate in Individual Support Plans.
  • Ensure staff supervision ratios as outlined in ISP.
  • Perform other duties as assigned by the Unit Director or RN.


REQUIREMENTS

  • Certified Nursing Assistant certification in good standing with the state nursing board.
  • High School Diploma or GED.
  • Valid driverโ€™s license and reliable transportation.
  • Ability to demonstrate safe driving skills per RHD standards.


ADDITIONAL QUALIFICATIONS

  • Physical ability to lift up to 100 pounds occasionally and 50 pounds frequently.
  • Ability to engage in crisis intervention and physical activity as required.
  • Strong commitment to person-centered care and RHD values.
