Description:
HIV Case Manager
Position Summary
We are seeking a dedicated and compassionate HIV Case Manager to support individuals living with HIV/AIDS by helping them access healthcare, support services, and community resources that promote improved health outcomes and quality of life.
In this role, you will serve as an advocate, educator, and care coordinator, empowering clients through knowledge, resource navigation, and individualized service planning. Responsibilities include intake and assessment, service planning, treatment adherence support, referrals, advocacy, reassessment, discharge planning, and HIV-related data collection and reporting. The HIV Case Manager plays a critical role in helping clients overcome barriers to care while supporting engagement in treatment and viral suppression goals.
Key Responsibilities
Client Intake & Assessment
  • Conduct intake and screening activities to determine client eligibility for services.
  • Complete comprehensive assessments evaluating medical, psychosocial, financial, insurance, and support service needs.
  • Assess barriers to healthcare access, medication adherence, and overall well-being.
  • Evaluate eligibility for HIV/AIDS insurance assistance programs and other financial assistance resources.
  • Conduct acuity assessments and identify unmet needs requiring intervention.
Care Planning & Service Coordination
  • Develop individualized service plans in partnership with clients to address identified needs, goals, and objectives.
  • Coordinate medical care, behavioral health services, and community-based support services.
  • Facilitate referrals to healthcare providers, housing resources, transportation programs, financial assistance programs, and other community resources.
  • Monitor service effectiveness and revise service plans as client needs change.
  • Advocate on behalf of clients to improve access to care and support services.
Treatment Adherence & Health Outcomes
  • Provide education and counseling regarding HIV treatment, medication readiness, and treatment adherence.
  • Support clients in achieving and maintaining viral suppression.
  • Educate clients on prevention strategies and applicable Michigan HIV disclosure laws.
  • Assist clients in accessing medical, dental, behavioral health, substance use, and other supportive services.
  • Maintain a working knowledge of HIV care systems and resources to improve patient outcomes.
Reassessment & Discharge Planning
  • Conduct periodic reassessments to identify new or changing client needs.
  • Evaluate progress toward goals and coordinate modifications to service plans as needed.
  • Collaborate with clients to develop discharge plans when services are no longer required.
  • Facilitate successful transitions to appropriate community resources and support systems.
Documentation & Reporting
  • Maintain accurate, timely, and complete documentation within the electronic medical record (EMR).
  • Collect, manage, and report required client data through the HRSA CareWare system.
  • Ensure documentation meets organizational, grant, and regulatory requirements.
  • Document all client interactions, service coordination efforts, and outcomes in accordance with program standards.
Professional Responsibilities
  • Collaborate effectively with multidisciplinary healthcare teams and community partners.
  • Maintain confidentiality and compliance with applicable federal, state, and organizational regulations.
  • Participate in program initiatives and quality improvement activities.
  • Perform other duties as assigned.
Qualifications
Education
Required:
  • Bachelor's degree in Social Work (BSW), Case Management, Human Services, Public Health, or a related field from an accredited college or university.
Certification & Training
Required:
  • Successful completion of HIV Counseling, Testing, and Partner Counseling and Referral Services (PCRS) Training and Case Management Training provided by the Michigan Department of Health and Human Services HIV/AIDS Prevention and Intervention Section (HAPIS) within one year of hire.
Experience
Required:
  • Two (2) years of experience in a patient care, healthcare, case management, or social service setting.

Preferred:
  • Experience working with individuals living with HIV/AIDS.
  • Experience with long-term case management and treatment adherence programs.
Knowledge, Skills & Abilities
Required
  • Knowledge of community resources and service navigation.
  • Understanding of health insurance programs and healthcare access.
  • Ability to assess and identify unmet client needs and barriers to care.
  • Strong advocacy, assessment, and problem-solving skills.
  • Excellent interpersonal and communication skills.
  • Ability to work effectively with individuals from diverse cultural, ethnic, and socioeconomic backgrounds.
  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
  • Experience with healthcare or social service documentation systems.
  • Ability to work independently while collaborating effectively within a multidisciplinary team.
Preferred
  • Experience supporting HIV treatment adherence and viral suppression initiatives.
  • Knowledge of Ryan White HIV/AIDS Program services and resources.
