Job description

This position delivers professional nursing care to patients in accordance with the established plan of care and Company policies and procedures. We respect your desire for independence and flexibility in your work and personal life and will design a FLEXIBLE SCHEDULE to meet your needs.

Service Area: Central Delaware

As an Addus Registered Nurse (RN), you will:

  • Skilled Nursing Home Health Case Management.
  • Perform Wound Care, Infusion Therapy, Patient Teaching, Medication Management.
  • Performing Supervisory Nursing Visits for Nonskilled Clients.
  • Maintain Accurate and Timely Documentation.
  • Provide Education and Guidance to Clients and Caregivers.
  • Support Quality Assurance and Risk Management Initiatives.
  • Communicate Effectively with Staff, Caregivers, Families, and Referral Partners.
  • Maintain a Collaborative Relationship with Clients, Families, Providers, and Care Team.
  • Able to Ensure Compliance with Federal and State Regulations, Medicare and Medicaid Requirements, HIPAA standards, and Company Policy
  • Performs other duties as assigned

REQUIREMENTS

  • Graduation from an accredited school of nursing and current RN licensure.
  • Home Health experience required
  • Familiarity with PointCare, Home Care Home Base is a plus.
  • Experience with OASIS preferred.
  • Must be able to travel locally in your own vehicle, and possess a valid driver's license and appropriate automobile insurance coverage.
