Riverside at Home in Warsaw is looking for an experienced and compassionate Home Health Physical Therapist to join our dedicated team!

In this rewarding role, you'll bring expert clinical skills and a personal touch to patients in the comfort of their homes-helping them restore function, improve mobility, relieve pain, and achieve greater independence. You'll perform evaluations and OASIS assessments, develop evidence-based plans of care, and deliver hands-on therapy that makes a measurable difference in daily living.

As a Staff Physical Therapist, you'll collaborate closely with an interdisciplinary team that values connection, innovation, and excellence in every aspect of patient care.

What You'll Do

  • Assess & Treat: Perform patient evaluations, determine PT diagnoses, and design goal-oriented plans of care based on best practices and current evidence.

  • Monitor Progress: Complete timely reassessments, including OASIS assessments, and update care plans in collaboration with physicians and care team members.

  • Collaborate & Lead: Build trusting relationships across the multidisciplinary team and actively participate in care conferences to ensure continuity of care.

  • Support & Supervise: Guide and mentor Physical Therapy Assistants and Home Health Aides, ensuring quality care and completion of required supervisory documentation.

  • Document & Plan: Prepare clear, comprehensive clinical and progress notes that demonstrate skilled intervention and support positive outcomes.

  • Drive Results: Carry out and adapt treatment plans to maximize patient recovery and independence, using evidence-based principles and compassionate care.

What You'll Bring

Education:

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy (required)

Experience:

  • 1 year of Physical Therapy experience (preferred)

Skills & Abilities:

  • Strong understanding of growth and development across the lifespan

  • Ability to assess patient needs, analyze data, and deliver age-appropriate care

  • Excellent communication and documentation skills

  • Collaborative spirit with a passion for patient-centered care

Licenses & Certifications:

  • Active Physical Therapy License (PT) - Virginia Department of Health Professions (required upon hire)

  • CPR/BLS Certification - AHA, ARC, or ASHI (required within 30 days)

  • Valid Driver's License (required; frequent travel using personal vehicle)
