Active Life Therapists is seeking a dedicated Physical Therapist to join our dynamic team in Greeley, Colorado. Work Environment

Based in Greeley, Colorado, our rehabilitation center provides an onsite setting that fosters collaboration among various healthcare professionals, including physicians, nurses, occupational therapists, and speech-language pathologists. We operate Monday through Friday, with rotating shifts on weekends, allowing our team members to maintain a healthy work-life balance while meeting the needs of our patient population.

Key Responsibilities

As a Physical Therapist at Active Life Therapists, you will:

  • Conduct comprehensive evaluations of patients to identify impairments, functional limitations, and disabilities stemming from injuries, illnesses, or other health challenges. Develop tailored treatment plans based on the evaluations and physician referrals.
  • Implement therapeutic interventions aimed at improving patients' mobility and functional capacity, continually assessing and adjusting treatments to maximize effectiveness.
  • Document all aspects of patient care including evaluations, treatment plans, progress notes, and discharge summaries in accordance with regulatory standards and institutional policies.
  • Supervise and mentor Physical Therapy Assistants (PTAs) and other support staff in delivering quality patient care, ensuring that the established treatment goals are met effectively and efficiently.
  • Collaborate with interdisciplinary teams to enhance patient outcomes, participating in case discussions and contributing to the continuous improvement of care delivery.
  • Maintain accurate records for billing purposes, ensuring that all patient charges are processed in a timely and professional manner.
  • Engage in professional development activities and contribute constructive feedback to the management team to foster a positive work environment conducive to growth and learning.
Patient Population / Client Focus

Your work will primarily focus on a diverse population of patients with various physical limitations due to injury, surgery, or chronic conditions. You will encounter individuals at different life stages, ranging from pediatric to geriatric patients. You will play an essential role in not only helping patients recover but also educating them on injury prevention and wellness practices to enhance their quality of life.

Qualifications

To be considered for this role, candidates must possess:

  • A Bachelor's Degree in Physical Therapy from an accredited institution.
  • A valid Physical Therapy license issued by the State of Colorado.
  • Strong written and verbal communication skills, essential for collaborating effectively with patients and the healthcare team.
  • Proficiency in using electronic health records and other necessary clinical software.
Preferred Experience
