Cedar Hill Regional Medical Center GW Health opened its doors in April 2025 as the first new full-service hospital in Washington, DC in more than 25 years! Integrating clinical care with existing community providers, Federally Qualified Health Centers (FQHC’s), The George Washington University Hospital, and our Urgent Care Center in Ward 8, this enhanced integrated delivery system will establish a robust network of care for all District residents, but more importantly, it will ensure residents of Wards 7 and 8 have access to high quality care in their community.  Our advanced hospital will offer 136 beds (with the ability to expand to 184 beds), a verified trauma center, adult and pediatric emergency departments, maternal health and newborn delivery, an ambulatory pavilion for clinic visits and other outpatient services, onsite 550-car parking garage, and a helipad for emergency transport.

We are seeking an experienced Case Management Associate (Full-Time, Days)
Our ideal candidate will be supporting the post-acute functions for patients who are in an inpatient or observation status.The Case Management Associate will facilitate the continuum of care and through-out process patients transitioning to the appropriate level of care, while ensuring the most cost effective and safe environment post-acute.

 

Job Duties/Responsibilities: 

  • Perform responsibilities consistent with hospital safety.  
  • Assist the case manager and healthcare team with the communication and collaboration of the post-acute planning functions including initiating referrals, home health, DME, arrange transportation, providing patient choice forms, and other skills.
  • Data entry of case related.
  • Provide clerical support to department as needed.
  • Transmit information regarding available beds to other Case Management Staff and Nursing Staff.
  • Fax referrals to facilities and follow-up on bed status and possible bed.
  • Deliver important message from Medicare to all Medicare patients prior to (4 to 48 hours prior to discharge) according to CMS guidelines.
  • Arrange ambulance transfers upon request with all pertinent information needed.
  • Arrange with Nursing Staff appropriate times to transfer patients by Ambulance/Medicare/Medicaid or Logisticare for Medicaid patients.
  • Call, manage, or fax to post-acute transition of care providers, necessary referral paperwork under the direction of the Case Manager for anticipated discharge.
  • Arrange in/out of state travel including but not limited to taxi, ambulance, and air ambulance.  
  • Fax DME necessary for referral paperwork and coordinate delivery of equipment upon request of the Case Manager.
  • Assist with obtaining medications through pharmaceutical companies for patients who qualify for assistance in getting their medication.
  • Inform patient of their right to choose when referral SNF, Home Health Care, etc.
  • Work closely with Case Managers, Social Workers, physicians, nurse managers, nursing supervisors, and other members of the healthcare team to achieve desired outcomes for his/her caseload.
  • Demonstrate and take responsibility for confidentiality/HIPPA, cultural/spiritual diversity and inclusion, and infection control/prevention.
