Job Description
OSF CompleteCare 55+ is an in-person and digital primary care model created to address the needs of adults 55+ who often face multiple chronic conditions, fragmented care, and barriers to timely access. Our clinics are designed to meet the unique health care needs of older adults through a personalized, team-based approach.
The Advance Practice Provider with OSF CompleteCare 55+ will support expanding access, and ensuring that patients receive team-based, coordinated care. This may include partnering with leadership, other providers, clinic and virtual care teams, and collaborating with cross-functionally key stakeholders to support consistent, efficient, and standardized processes. OSF CompleteCare 55+ features patient-provider visits in-person or virtually; integration of existing OnCall Digital tools such as Remote Patient Monitoring, Chronic Care Management,

    • Range shown should be used as an estimate and can be impacted by many factors including but not limited to the critical need of the position, overall experience and qualifications, community need, and other considerations. Additional compensation components may be applicable such as bonuses or incentives which may or may not be based on metric achievement.

  • Master's or Doctoral degree from an accredited Nurse Practitioner program or graduate of an accredited Physician Assistant program
  • Current Illinois APN or PA license (or eligibility to obtain)
  • Board certification required
  • Previous primary care, family medicine, internal medicine, geriatrics, or senior-focused care experience required
  • Strong communication, collaboration, and patient engagement skills
