Responsibilities:

  • Conducting initial and ongoing assessments of clients to determine their care needs.
  • Developing, implementing, and updating client care plans in collaboration with physicians and the home health team.
  • Providing direct skilled nursing care to clients of varying ages and acuity levels.
  • Coordinating care with families, caregivers, and other healthcare providers.
  • Medication Management: Ensuring proper medication administration and reconciliation.
  • Client Monitoring: Regularly monitoring clients' conditions and reporting any changes to the appropriate personnel.
  • Providing education and support to clients, families, and caregivers.
  • Ensuring accurate and timely documentation of client care and progress.
  • Ensuring the safety needs of clients are addressed through home environment assessments and individualized abuse prevention plans.
  • Providing clinical oversight, delegation, and leadership to other licensed and unlicensed personnel.
  • Documentation: Maintaining accurate and up-to-date client records.
  • Compliance: Adhering to all relevant regulations, including OSHA and HIPAA.
  • Managing your schedule effectively and participating in the on-call phone rotation on occasion.
Qualifications:

  • Currently licensed and unrestricted as a Registered Nurse (RN) in the State of Minnesota and Wisconsin.
  • Certified in CPR and have a negative TB skin test or chest x-ray.
  • Minimum of one (1) years of experience in a health care facility or home care setting. Home Health Experience: Two or more years of clinical experience, with one year in a home health agency, primary care clinic, or health facility, is preferred.
  • Driving: A valid driver's license and reliable transportation are necessary.
  • Demonstrates critical thinking skills and the ability to make appropriate clinical judgments.
  • Excellent written and oral communication skills.
  • Proficiency in using computer systems and electronic charting is essential.
