What You'll Do
  • Complete evaluations, reassessments, and discharge visits
  • Develop and manage individualized plans of care
  • Provide skilled physical therapy services in patients' homes
  • Complete Starts of Care and progress documentation
  • Educate patients and caregivers on mobility, safety, and recovery strategies
  • Document electronically using WellSky on a company-issued tablet
  • Collaborate with physicians and interdisciplinary team members
Service Area

Based out of Blacksburg, you'll serve patients throughout the New River Valley, including:

  • Blacksburg
  • Christiansburg
  • Radford
  • Pulaski
  • Floyd
  • Montgomery County
  • Giles County
  • Surrounding communities

Assignments are typically scheduled geographically to support efficient travel and work-life balance.

Qualifications
  • Active Virginia Physical Therapist License Required
  • Minimum One Year of Physical Therapy Experience
  • Home Health Experience Preferred
  • OASIS Experience Preferred but Not Required
  • Valid Driver's License and Reliable Transportation
  • Comfortable Working Independently in Patients' Homes
