Job Title: Case Manager
Department: Adult Workforce Development
Program: Franklin County Trades Academy
Reports to: Program Manager
Position Supervised: None
Employment Class: Regular/Full-Time/Exempt
Workdays: Monday - Friday
Customary Hours: 8:30a - 5p(*Hours are extended when workload is increased.)
Position Summary:
Case Managers directly support participants in the Franklin Trades Academy, assisting them in overcoming barriers and tracking their progress through training and employment. This role emphasizes participant advocacy and collaboration with unions and employer partners to ensure successful outcomes.
Duties & Responsibilities:
Participant Support:
  • Conduct weekly and monthly follow-ups with participants to monitor progress and address challenges.
  • Provide individualized coaching to help participants navigate barriers such as transportation and housing.
  • Facilitate group check-ins to foster community and accountability among participants.

Collaboration:
  • Work closely with union and employer partners to address workplace concerns and support participant retention.
  • Coordinate with the intake coordinator to ensure smooth transitions for new participants.

Compliance:
  • Maintain detailed and accurate case notes for all assigned participants.
  • Submit weekly reports on participant progress and service delivery metrics.
  • Ensure all participant records are up to date and audit ready.

Other duties as assigned.
Experience and Skills
Education and Experience:
Education:
  • Bachelor's degree in social work, counseling, or a related field is preferred
  • May substitute years of experience for education
  • Willingness and ability to obtain other certifications as necessary

Experience:
  • 3-5 years of experience in case management or workforce development

  • Ability to read, write, and speak English proficiently
  • Effective and professional verbal and written communication skills
  • Strong interpersonal and customer service skills
  • Ability to work with individuals from all backgrounds with varying needs

  • Experience with videoconferencing systems (e.g., Webex, Zoom, Teams, etc.)

  • Ensures accuracy, efficiency, and consistency across all tasks
  • Works independently with the ability to handle multiple tasks
  • Manages workload effectively and delivers results in a timely manner

Work Environment:
  • While performing the duties of this job, the employee is regularly required to talk, hear, and see.
  • The employee must occasionally lift and/or move up to 50 pounds.
  • The noise level in the work environment is usually moderate to low.
  • The employee is required to occasionally travel to different work sites and community events as needed.
