Job description

A-Line Staffing is now hiring an RN Case Manager. This will be Full Time.

  • Schedule – M-F 8am-5pm – FULLY REMOTE
  • Pay Rate- $34.36 an hour paid weekly
  • Benefits – active after 90 days of employment

Key Responsibilities

  • Conduct comprehensive telephonic assessments of member health needs, eligibility, and benefits
  • Develop, implement, and evaluate individualized care plans
  • Apply clinical guidelines, policies, and regulatory standards to determine appropriate care pathways
  • Coordinate services across providers, programs, and internal/external resources
  • Support members in accessing appropriate care and benefits
  • Monitor outcomes and adjust care plans as needed
  • Ensure compliance with regulatory, accreditation, and organizational standards
  • Document all interactions and case activity accurately in electronic systems
  • Collaborate with interdisciplinary care teams to optimize outcomes

Required Qualifications

  • RN License in OH or Compact RN License
  • BSN required (or equivalent as specified in role requirements)
  • Minimum 3 years of clinical practice experience in case management or home health or discharge planning
  • CCM Preferred
