ROLE SUMMARY:

The Patient Access & Care Team LPN (PACT LPN) plays a critical role in supporting patient care by managing electronic communications within the electronic health record (EHR). This role is responsible for reviewing, prioritizing, and responding to patient messages, refill requests, and other provider-directed communications in a timely and clinically appropriate manner.

Working in close collaboration with providers, the triage RN, and the broader care team, the LPN helps ensure safe, efficient, and patient-centered communication and follow-through. This position requires strong clinical judgment within LPN scope, excellent written communication skills, attention to detail, and the ability to manage a high volume of tasks in a fast-paced, team-based environment.

This role serves a diverse and often medically and socially complex patient population and requires a strong commitment to delivering compassionate, respectful, and equitable care in every interaction. The ideal candidate demonstrates professionalism, flexibility, and a positive, team-oriented mindset, and is energized by working collaboratively to build and support a strong, mission-driven team culture. As this is an evolving role, the LPN must be adaptable and open to ongoing workflow development and continuous improvement.

LPNs in this role are expected to practice in accordance with the Ohio Nurse Practice Act and within their licensed scope.


KEY RESPONSIBILITIES:

In-Basket Management & Patient Communication

  • Manages incoming EHR in-basket messages, including patient portal (MyChart) messages, refill requests, and internal care team communications.
  • Reviews and prioritizes messages based on clinical urgency and established protocols.
  • Responds to patients within scope using approved protocols, standing orders, and provider guidance.
  • Routes messages appropriately to providers, triage RN, or other team members when escalation is needed.
  • Ensures timely follow-up and closure of messages to support continuity of care.

Clinical Support & Triage Collaboration

  • Performs focused assessments of patient-reported symptoms via electronic communication or phone, within LPN scope.
  • Collaborates closely with the triage RN for higher-acuity concerns or when clinical guidance exceeds LPN scope.
  • Supports providers by preparing and pending orders for review (e.g., refills, labs, routine orders) per protocol.
  • Assists in communicating test results and care plans to patients as directed by providers.

Care Coordination & Population Health Support

  • Identifies care gaps, overdue health maintenance, and needed follow-up based on in-basket interactions.
  • Coordinates appointments, referrals, and services as needed to support patient care plans.
  • Reinforces patient education related to medications, chronic conditions, and preventive care.
  • Connects patients to community resources when appropriate.

Documentation & Compliance

  • Documents all patient interactions accurately, clearly, and in a timely manner within the EHR.
  • Maintains compliance with organizational policies, clinical protocols, and regulatory requirements.
  • Adheres to privacy and confidentiality standards in all communications.

Quality, Teamwork & Operations

  • Meets established performance expectations for in-basket turnaround times, accuracy, and quality.
  • Participates in team huddles, trainings, and ongoing workflow optimization efforts.
  • Contributes to quality improvement initiatives related to patient communication, access, and care coordination.
  • Assists with onboarding and training of new team members as needed.
  • Performs other duties as assigned.

Qualifications

  • Completion of an accredited Licensed Practical Nursing program required.
  • Current unrestricted Ohio LPN license required.
  • Active BLS certification required.
  • Two years of clinical experience required; experience in primary care, triage, care coordination, or FQHC setting strongly preferred.
  • Experience with Epic or other electronic health record(s) strongly preferred
  • Strong written communication, critical thinking, and organizational skills required.
  • Ability to manage high-volume electronic workflows with accuracy and efficiency.
  • Bilingual (Spanish-speaking) skills highly preferred.

Work Environment

  • Primarily computer-based, high-volume workflow environment.
  • Fast-paced setting requiring frequent task-switching and prioritization.
  • May require travel to LLCHC sites for coverage, training, or team collaboration.
  • Work location may vary based on organizational needs.
